Health Services-Requirements for Admission

Lee University complies with the State of Tennessee in requiring proof of two immunizations for Measles, Mumps, and Rubella (MMR).Also required is proof of two immunizations for Varicella (chicken pox).Alternatives to the varicella immunization series is verification by a licensed medical provider that you have had the disease, or by having a blood titer for varicella drawn and tested to show immunity.

Both immunizations are mandatory and should be completed prior to registration.Applicants shall submit with the necessary Health Clinic forms, a legitimate “Record of Immunization” that can be obtained from their local health department, the high school of graduation, or the private physician used for immunizations.These forms should be submitted to the Lee University Health Clinic or to the University Admissions Office prior to registration.Applicants can also download the “Immunization Requirements” and “Immunization Form” from this webpage.This form must be signed by a licensed medical provider.Parent or student signatures are not acceptable!

International students are also required to show proof of testing for TB (tuberculosis).If proof of testing is not submitted, this will be required at registration.
