Federal Direct Loan Forms
The MPN and Entrance Counseling are required for any student who is a first time borrower of Federal Direct Loans. These must be completed prior to receiving these funds. The Exit Counseling is required upon graduating or dropping below half-time enrollment.

Federal Direct Subsidized and Unsubsidized Master Promissory Note (MPN)
New borrowers must complete a Master Promissory Note prior to receiving loan funds. Students can complete this online at www.studentloans.gov. You will need you Federal Pin number to complete the MPN. If you do not remember your pin number you may retrieve it at www.pin.ed.gov. Your funds will be disbursed electronically to your student account.

Direct Loan Entrance Counseling
The interview process provides valuable information about the Federal Direct Loan Program. It is a Federal requirement that recipients must complete this step prior to receiving funds from this program. Entrance Counseling can be completed online at www.studentloans.gov.

Direct Loan Exit Counseling
Federal regulations require all students who borrowed Direct Loan funds to complete exit counseling when graduating or dropping below half-time enrollment. This must be completed even if you intend to re-enroll the next semester. This is required for graduation and must be completed before a diploma will be issued. You will need to complete the online exit counseling at www.studentloans.gov.

Federal Perkins Student Loan Forms
The Entrance Counseling form is only required for first time borrowers of a Federal Perkins Loan. The Exit Counseling form is required upon graduating or dropping below half-time enrollment.
Perkins Master Promissory Note
This is required for any student who is a first time borrower of a Federal Perkins Loan. This must be completed prior to receiving these funds. You will complete this on-line by clicking “Students and Borrowers enter here,” entering Lee’s information, and then following the instructions listed.
Perkins Loan Exit Counseling
Federal regulations require all students who borrowed Perkins funds to complete an exit counseling when graduating or dropping below 1/2 time enrollment. This is required for graduation and must be completed before a diploma will be issued.

Federal Direct Plus Loan Forms
These forms must be completed by the parent or graduate borrower.

Federal Direct Plus Loan Master Promissory Note (MPN)

New Parent or Graduate school borrowers must complete a Master Promissory Note and Plus application prior to receiving loan funds. Parents can complete this on-line at www.studentloans.gov. The funds will be disbursed electronically to your student’s account.

Grad Plus Loan Entrance Counseling
The interview process provides valuable information about the Federal Graduate Plus Loan Program. It is a Federal requirement that recipients must complete prior to receiving funds from this program.
Grad Plus Loan Exit Counseling
Federal regulations require all students who borrow Graduate Plus funds to complete exit counseling when graduating or dropping below half-time enrollment.

Alternative Loan Options
The Link below will take you to a lender list and you will apply directly on the lender’s website for this loan. Alternative Loans are a non-federal loan. They are a credit-based loan that may be used as a supplemental resource for paying college costs. You should exhaust all federal aid opportunities first. Terms and conditions of Federal aid programs are usually more favorable than alternative loans.
Alternative Loan Lender List and Promissory Note
Alternative loans will require an applicant to accept an Application Disclosure, Approval Disclosure, Final Disclosure, and submit a completed Self-Certification Form to the lender. Please be aware that this will take longer to process and disburse to your account. Students can complete this online with one of the primary lenders in this link. If you choose one of these lenders, your funds will be disbursed electronically to your student account. Please note that you do not have to use a lender on this list. You may apply directly on the website of the lender of your choice.
