Verification Forms
Not all students are required to complete this form.
Some students are selected by the Department of
Education for verification in order to establish a
completion accuracy rate. If you are required to
complete this process, you will be notified on the
Student Aid Report sent to you from the Department of
Education and the Lee University Financial Aid Office
will send you a letter requesting the required forms.
Once in verification, you are required to submit a
signed copy of 2007 tax returns for the student and
parents (in the case of dependent students) as well as
the appropriate verification form listed below.
Certification for Electronic Corrections

This form is only required in the event that
corrections are made to the federal application. Both
the student and at least one parent (if a dependent
student) must sign this form.
Federal Satisfactory Progress Appeal
(Need

This form is only required for students who did not
maintain satisfactory academic progress and received
Federal Aid during their
last period of enrollment at Lee University.
Tennessee Satisfactory Progress Appeal
(Need

This form is only required for students who did
not maintain satisfactory academic progress and received
the Tennessee HOPE Scholarship during their last period
of enrollment at Lee University.
Tennessee Hope Enrollment Change Request Form
(Need

Students receiving the Tennessee HOPE
Scholarship must receive prior approval before changing
from full-time to part-time. |
