Application Requirements Checklist

     c  Completed application, postmarked by 15 April 2008

     c  Copy of one of the following: Birth Certificate or valid state-issued ID
           that states birth date

     c Two (2) copies of a standard 15-20 minute audio CD of excellent
           recording quality, marked with initials ONLY.  There is to be no
           talking or other identifying information on the CD.  CDs become
           the property of Lee University and are not returnable.

     c A $50 (US funds) non-refundable application fee. Check should be made payable to Lee University.

          All items must be included in a single envelope, postmarked by 15 April
           2008,and mailed to:
